ACI Alliance Task Force Guidelines for Clinical Assessment and Management of Cochlear Implantation in Children and Adults with Bilateral & Single-Sided Deafness 


Ear & Hearing, The Official Journal of the American Auditory Society, has published the first three papers by ACI Alliance Task Forces on CI candidacy and management. The Task Forces were assembled to provide clarity on candidacy determination for adults (bilateral deafness and SSD) and children (bilateral deafness and SSD).

We are grateful for the important collaborative work of the task forces in helping promote broader understanding of candidacy determination and management in these populations.


American Cochlear Implant Alliance Task Force Guidelines for Clinical Assessment and Management of Cochlear Implantation in Children With Single-Sided Deafness

With greater recognition of the importance of bilateral hearing in children for language development and overall academic success, single-sided deafness is now an accepted candidacy indication for cochlear implantation. This paper reviews both the current evidence and guidance for referral to a cochlear implant team: a 3-frequency pure-tone average of 60 dB HL or poorer or an aided speech intelligibility index poorer than 0.65 in the ear to be implanted. This paper also details evaluation and management of children with single-sided deafness who receive cochlear implants, including suggestions for spatial hearing assessment and subjective questionnaires.

Current criteria for cochlear implantation in children has shifted from the historical requirement of bilateral profound sensorineural hearing loss to a more flexible and broader interdisciplinary approach.

This manuscript combines global research evidence with clinical expertise across multiple areas to promote a 50/70+ guideline for pediatric cochlear implant referrals: 50% word recognition, audiometric thresholds of at least 70 dB HL, and poor functional performance (e.g., less than month-over-month progress for speech, language, or hearing; poor well-being or quality of life).

American Cochlear Implant Alliance Task Force Guidelines for Clinical Assessment and Management of Cochlear Implantation in Adults With Single-Sided Deafness

The importance of binaural hearing benefits not only children, but also adults. This paper probes the literature on single-sided deafness in adults relative to its effect on communication, social interaction, work opportunities, and quality of life to develop guidelines for when clinicians should refer patients for a cochlear implant evaluation. The task force guidelines outline audiologic (i.e., 4-frequency pure-tone average of at least 80 dB in the ear to be implanted plus 4-frequency pure-tone average no better than 55 dB HL in the contralateral ear), medical, and functional performance as measured by subjective questionnaires.

American Cochlear Implant Alliance Task Force: Recommendations for Determining Cochlear Implant Candidacy in Adults

Due to a variety of factors, only a small percentage of adults who could benefit from a cochlear implant (CI) receive one. Indications for cochlear implantation have expanded but there is variability in how providers are assessing patients and whether they are using up-to-date indications. This paper reviews the current evidence for determining appropriate CI referrals for adult patients with bilateral hearing loss, emphasizing the importance of evaluating each ear individually, utilizing and using a “revised 60/60 rule.” The paper was developed by the Adult Cochlear Implant Candidacy Task Force of the American Cochlear Implant Alliance relying upon a literature review and includes recommendations for a standardized protocol for testing CI candidates.
