Listening @ ACI Alliance
Listening is a vehicle for us to share information on cochlear implants and hearing loss with hearing healthcare providers, adult CI recipients (and those exploring CI), parents and family members interested in CI, educators who work with deaf and hard of hearing children, and anyone wanting to learn more about cochlear implants.
ACI Alliance is a not-for-profit organization created with the purpose of eliminating barriers to cochlear implantation by sponsoring research, driving heightened awareness and advocating for improved access to cochlear implants for people of all ages. There is a wealth of information on our website.
Our work at ACI Alliance follows our mission encompassing research, advocacy, and awareness. Accordingly, Listening covers all three.
We encourage you to share sign-up details for this free e-newsletter with patients, family members, colleagues or anyone who may be interested.

Cochlear Implants Now More Accessible to Older Adults

Until recently, candidacy criteria for Medicare beneficiaries who may benefit from cochlear implantation (CI) was more stringent than the FDA guideline, typically followed by private health insurers. The narrow Medicare criteria were changed on September 26, 2022, when the Centers for Medicare & Medicaid Services (CMS) published its final rule under a process called a National Coverage Determination (NCD) to expand coverage for cochlear implants under Medicare. The CMS action facilitates an important improvement in access to CI care for older adults and confirms the benefits of broader criteria for other insurers.

Understanding Foreign-Accented English

Many people with hearing loss have difficulty understanding the speech of people with a foreign accent. This listening challenge is even present in favorable communication conditions, such as in a quiet environment and with speechreading.

Adult
aural rehabilitation is a type of therapy that focuses on hearing and
communication when a person has hearing loss. Aural rehabilitation for adults
promotes improved communication by teaching participants to take charge of
their hearing loss.

Global report on health equity for persons with disabilities
The World Health Organization (WHO) recently released the Global report on health equity for persons with disabilities, highlighting the barriers persons with disabilities face in achieving an equitable standard of health across the globe. The report shows that despite some recent progress there are still many inequities and poor health outcomes are often the result of unfair conditions faced by persons with disabilities in all facets of life, including in the health system itself.

Second-Time Hearing Handicap

The cochlear implant, as the most successful neural prosthesis, has restored functional hearing for over one million people worldwide. 1 On average, the implant improved sentence recognition from the pre-surgical performance of 24% correct to the post-surgical performance of 77%, but the individual variability is large. 2 Donna and Feng represent the range of the second-time hearing handicap in actual cochlear implant users. Unlike the first-time hearing handicap, which, made both Feng and Donna rely on lipreading only, the second-time hearing handicap is less apparent but real.

Medicare Expansion of CI Candidacy Is ‘Life-Changing’ for Older Patients
With Medicare’s newly expanded cochlear implant (CI) coverage, more adults 65 and older are eligible to receive the device. And that can be life-changing, say some of the people who received CIs in studies that led to the expanded criteria.

The Advanced Research Projects Agency for Health (ARPA-H) is currently seeking Program Managers. Program Managers (PMs) have a critically important role in ARPA-H, and the choice of these PMs will determine to a large extent the agenda of the agency.

Passage of the Early Hearing Detection and Intervention Act
Congress passed and President Biden signed into law the 2022 Early Hearing Detection and Intervention (EHDI) Reauthorization. The new law includes a requirement for a GAO study on how early intervention information is collected and provided via websites and other formats, and ways to address historically underserved communities. ACI Alliance research called attention to the need for oversight of these legislatively mandated information sources. The FY2023 Omnibus Act also included language that encourages the Health Resources Services Administration to work with partners to advance awareness about the wide range of modalities and hearing technologies available for children who are deaf and hard of hearing and specifically mentions cochlear implants.

AZ State Champion Worked to Extend Medicaid Coverage for CI in Arizona to Adults
Arizona State Champ, Dr. Shawn Stevens (second from left), testified before the Arizona Senate Health Committee on expanding Medicaid coverage to cover CI for adults. Dr. Stevens has worked on this effort over the past few years and the bill passed the first crucial step. ACI Alliance continues to support this and efforts in other states to expand Medicaid coverage for adults (along with children) in all 50 states. Read more about the incredible work the state champs program has done across the states to increase awareness and help change coverage.

Lehigh Valley Health Network Shares ACI Alliance Resources
Lehigh Valley Health Network features ACI Alliance as a resource on their website about hearing loss, directing patients interested in cochlear implants to resources on the ACI Alliance website through a hotlink. Sharing the ACI Alliance website is a great way to expand public understanding of CI and we urge clinics and hospitals to consider this for their own websites.

Cochlear Implant Awareness in the United States: A National Survey of 15,138 Adults
Hearing loss is increasingly recognized as a chronic disease that warrants treatment. Depression, social isolation, loneliness, and poorer cognitive performance have all been linked to untreated and undertreated adult-onset hearing loss. A significant subset of the patient population with hearing loss is inadequately rehabilitated by hearing aids alone and may benefit from cochlear implantation. Yet, it is estimated that less than 10% of those who qualify have received implants to date.

Population and Clinical Database Resources
The NIDCD Epidemiology and Statistics Program team has compiled a list of databases that may be useful to investigators interested in NIDCD mission areas. Epidemiological studies and surveys provide a wealth of data related to the prevalence, incidence, risk factors, and preventive interventions for diseases related to hearing, balance, taste, smell, voice, speech, or language. Much of the data from these studies are publicly available for scientists to leverage as a part of their own research. Check out NIDCD’s new Population and Clinical Database Resources webpage to browse example databases by topic, read about the study cohorts for each dataset, and learn how to access the data for your own research.
